Application Forms

• Landlord's Application for Termination of Tenancy and Rental Arrears (pdf)- This application form is to be used by Landlord's when their tenant is still residing in the rental premises.
  
  
• Landlord's Application for Damages (pdf)- This application form is to be used by Landlord's when their tenant no longer resides in the rental premises.
  
  
• Tenant's Application (pdf)- This application form is to be used by Tenants.
  

If the rental premises in question is located in Northern or Central Alberta you may file your application with the RTDRS by fax. You will need to complete and submit the Fax Filing Payment Information Form (pdf) so that the RTDRS can process the $75.00 filing fee. The above application packages can be printed on standard 8.5 X 11 inch paper. They can also be obtained at the RTDRS office (see contact information) or at the courthouse closest to you.
