How to apply

Request a correction to an Alberta record.

You can request a correction by either sending your amendment request to Vital Statistics or visiting an Alberta registry agent office.

What’s important to know

It usually takes 8 to 10 weeks to complete an amendment.

  1. Complete the form 
  2. Send the Request to Amend form

Through a registry agent

  1. Complete the form 
  2. Drop off the Request to Amend form 
    • take the completed Request to Amend form to a registry agent office
    • the registry agent may collect the amendment fee and also charge a service fee

Find a registry agent

The registry agent will forward your amendment request to Vital Statistics. A registry agent may charge a fee for their service.

Vital Statistics will contact you regarding the required documents, evidence and applicable fees.
